Adenosine stress protocol is a standardized pharmacological method for inducing coronary hyperemia during cardiac imaging studies. Adenosine is infused intravenously at 140 mcg/kg/min for 4-6 minutes, causing maximal coronary vasodilation through adenosine receptor activation. This protocol is commonly used with MRI, PET, and SPECT imaging to detect coronary artery disease by revealing perfusion abnormalities. The protocol requires careful monitoring for side effects including chest pain, dyspnea, AV block, and bronchospasm. Contraindications include severe asthma, high-grade AV block, and recent caffeine intake, which blocks adenosine receptors.
