Coronary artery anomaly classification systems organize congenital coronary variations into standardized categories to facilitate communication, risk assessment, and management decisions. The most widely used classification divides anomalies into: 1) anomalies of origination and course (including anomalous aortic origin, single coronary artery, and coronary arising from pulmonary artery); 2) anomalies of intrinsic coronary anatomy (myocardial bridges, coronary fistulae); and 3) anomalies related to congenital heart disease. Further subcategorization considers whether the anomalous vessel courses between the great vessels (potentially malignant) or takes a benign pathway. This classification helps determine clinical significance, need for further evaluation, and appropriate management strategies.
