Full Definition

Aorto-ostial lesions occur at the junction between the coronary artery ostium and the aortic wall, representing a challenging subset of coronary disease due to their location and characteristics. These lesions often contain fibrous and calcific tissue extending from the aortic wall and are prone to elastic recoil after balloon angioplasty. Intervention typically requires precise stent positioning with the proximal edge exactly at the ostium to avoid geographic miss while preventing stent protrusion into the aorta. The procedure often necessitates aggressive lesion preparation, high-pressure stent deployment, and may benefit from intravascular imaging guidance. Aorto-ostial lesions have higher rates of restenosis and may require larger profile equipment due to calcification and fibrosis.
