Atherogenic dyslipidemia is a specific pattern of lipid abnormalities that significantly increases cardiovascular risk, particularly prevalent in patients with insulin resistance, metabolic syndrome, and type 2 diabetes. The classic triad consists of elevated triglycerides, reduced HDL cholesterol, and a shift toward small, dense LDL particles that are more atherogenic than large, buoyant LDL particles. This lipid phenotype is often present even when total and LDL cholesterol levels appear normal, making it a particularly insidious cardiovascular risk factor. The pattern results from increased hepatic VLDL production and altered lipoprotein metabolism associated with insulin resistance. Treatment typically involves lifestyle modifications, statin therapy, and sometimes fibrates or other triglyceride-lowering agents.
