Beta-cell mass refers to the total volume or number of insulin-producing beta cells within the pancreatic islets of Langerhans, representing the functional reserve for insulin secretion. In healthy individuals, beta-cell mass can adapt to changing insulin demands through processes of replication, neogenesis, and apoptosis. Progressive loss of beta-cell mass is a hallmark of both type 1 and type 2 diabetes, though through different mechanisms. In type 1 diabetes, autoimmune destruction leads to near-complete beta-cell loss, while in type 2 diabetes, beta-cell mass gradually declines due to chronic metabolic stress, oxidative damage, and apoptosis. Preservation or regeneration of beta-cell mass represents a major therapeutic goal in diabetes research, with potential interventions including immunomodulation, beta-cell replacement, and regenerative therapies.
