Epicardial echocardiography involves direct placement of a sterile ultrasound transducer on the surface of the heart during cardiac surgery. This technique provides superior image quality compared to transesophageal approaches due to the absence of intervening structures. It is particularly valuable for assessing valve repairs, detecting residual shunts, and evaluating ventricular function immediately after surgical intervention. The technique requires careful coordination between the anesthesiologist, surgeon, and echocardiographer to avoid interference with surgical manipulation.
