T1 mapping is an advanced cardiac MRI technique that provides pixel-by-pixel quantification of T1 relaxation times, allowing assessment of myocardial tissue composition and pathology. Native T1 mapping can detect diffuse myocardial fibrosis, edema, and infiltrative diseases without requiring contrast administration. Post-contrast T1 mapping enables calculation of extracellular volume fraction (ECV), which quantifies the interstitial space and provides a measure of diffuse fibrosis. This technique is particularly valuable for detecting subtle myocardial changes in cardiomyopathies, cardiac amyloidosis, and other infiltrative diseases that may not be apparent on conventional imaging. T1 mapping provides objective, reproducible measurements that can track disease progression.
