Giant cell arteritis of coronary arteries is a rare manifestation of systemic giant cell arteritis (temporal arteritis) that specifically involves the coronary circulation. This granulomatous vasculitis primarily affects medium and large-sized arteries and is characterized by inflammatory infiltrates containing lymphocytes, epithelioid cells, and characteristic multinucleated giant cells. The inflammatory process typically involves the media and adventitia, with destruction of elastic fibers and smooth muscle cells. In coronary arteries, this can lead to stenosis, occlusion, or aneurysm formation, potentially causing myocardial infarction. The condition most commonly affects patients over 50 years of age and may be associated with polymyalgia rheumatica.
