ISHLT grading

The standardized histological classification system for cardiac allograft rejection developed by the International Society for Heart and Lung Transplantation.
Full Definition
ISHLT grading provides standardized criteria for assessing cardiac allograft rejection, most recently revised in 2004. The system grades acute cellular rejection from 0R (no rejection) to 3R (severe rejection) based on the degree and distribution of inflammatory infiltrates and associated myocyte damage. Grade 1R shows interstitial and/or perivascular infiltrate without myocyte damage, 2R has one focus of infiltrate with associated myocyte damage, and 3R shows diffuse infiltrates with multifocal myocyte damage and hemorrhage. The system also includes criteria for antibody-mediated rejection. Standardized grading ensures consistency across transplant centers worldwide.
