Lambl excrescences are small, thread-like projections that commonly occur on the ventricular surface of the aortic valve and less frequently on other cardiac valves. These structures consist of endothelium-covered connective tissue and are generally considered benign, age-related changes. They typically measure less than 1 cm in length and have a characteristic filamentous appearance. While usually incidental findings, large or numerous excrescences may rarely be associated with embolic events or may be mistaken for vegetations in the setting of suspected endocarditis.
