Löffler endocarditis represents the cardiac manifestation of hypereosinophilic syndrome, characterized by eosinophil-mediated endocardial damage leading to thrombosis and progressive fibrosis. The condition progresses through three stages: acute necrotic (eosinophilic infiltration and myocyte damage), thrombotic (mural thrombus formation), and fibrotic (endocardial scarring and restriction). The fibrotic stage leads to obliteration of the cardiac apex and posterior mitral leaflet immobilization. Pathologically, there is dense eosinophilic infiltrate, endocardial thickening, and organized thrombus. The condition most commonly affects tropical regions and is associated with parasitic infections, though it can occur in temperate climates with other causes of hypereosinophilia.
