ASCVD risk equivalents are clinical conditions that carry a cardiovascular event risk comparable to patients with established atherosclerotic cardiovascular disease, typically >20% 10-year risk. These conditions include diabetes mellitus in certain populations, chronic kidney disease with reduced eGFR, and familial hypercholesterolemia. Recognition of risk equivalents allows clinicians to initiate high-intensity preventive therapies without requiring formal risk calculation. The concept helps identify patients who benefit from aggressive lipid-lowering therapy regardless of calculated risk scores.
