bifurcation lesion

Coronary stenosis involving a vessel branch point, classified by the Medina system based on involvement pattern.

Full Definition

A bifurcation lesion is a coronary stenosis occurring at or involving a coronary artery branch point where a side branch with diameter ≥2.0mm originates from the main vessel. These lesions are classified using the Medina classification system, which describes stenosis presence in the proximal main vessel, distal main vessel, and side branch using a three-digit notation. Bifurcation lesions present unique technical challenges for percutaneous intervention due to potential side branch compromise, increased procedural complexity, and higher restenosis rates compared to non-bifurcation lesions.

Usage

Usage note: Always specify Medina classification when describing bifurcation lesion anatomy.

In Context

  • "The LAD-diagonal bifurcation lesion was classified as Medina 1,1,1 requiring complex intervention strategy." — Procedural note
  • "Provisional stenting remains the preferred approach for most bifurcation lesions with non-significant side branch stenosis." — Treatment guideline
