SYNTAX II score

Updated risk stratification tool that incorporates clinical variables with angiographic complexity to guide revascularization decisions.

Full Definition

The SYNTAX II score represents an evolution of the original SYNTAX score by incorporating patient-specific clinical variables alongside angiographic complexity to provide individualized mortality predictions for patients with complex coronary disease. This scoring system combines the anatomical SYNTAX score with clinical factors including age, creatinine clearance, left ventricular ejection fraction, presence of unprotected left main disease, peripheral vascular disease, and female gender. The SYNTAX II score generates separate mortality predictions for percutaneous coronary intervention (PCI) and coronary artery bypass grafting (CABG), helping clinicians and patients make evidence-based treatment decisions. This tool is particularly valuable in heart team discussions for patients with three-vessel disease or left main coronary disease.

Usage

Usage note: Always use Roman numeral 'II' to distinguish from the original SYNTAX score.

In Context

  • "The SYNTAX II score favored surgical revascularization with a 4-year mortality advantage over PCI." — Heart team discussion note
  • "Using the SYNTAX II calculator, the predicted mortality difference between CABG and PCI was minimal." — Multidisciplinary conference report
