Thin-Cap Fibroatheroma (TCFA) represents the most dangerous type of coronary atherosclerotic plaque, characterized by a large necrotic lipid core covered by a thin fibrous cap typically less than 65 micrometers thick. These plaques are highly vulnerable to rupture due to inflammatory cell infiltration and proteolytic enzyme activity that weakens the fibrous cap. TCFA identification through advanced imaging modalities like OCT or NIRS is crucial for risk stratification, as these lesions are responsible for the majority of acute coronary syndromes even when not causing significant luminal stenosis.
