Full Definition

Coronary flow velocity reserve (CFVR) is a hemodynamic parameter that quantifies the capacity of the coronary circulation to increase blood flow in response to increased metabolic demand or pharmacological stimulation. It is calculated as the ratio of peak coronary flow velocity during maximal hyperemia (typically induced by adenosine or dipyridamole) to the baseline flow velocity at rest. CFVR reflects the functional status of both epicardial coronary arteries and the coronary microcirculation, making it valuable for detecting coronary artery disease and assessing microvascular function. Normal CFVR values are typically greater than 2.0, with lower values indicating impaired coronary flow reserve. This measurement is commonly obtained using Doppler echocardiography or invasive Doppler wire techniques during cardiac catheterization.
