A transannular patch is a surgical technique used to relieve severe right ventricular outflow tract obstruction by extending a patch across the pulmonary valve annulus, effectively destroying valve competency but eliminating stenosis. This approach is commonly employed in tetralogy of Fallot repair when the pulmonary annulus is severely hypoplastic and cannot accommodate adequate forward flow. The patch typically extends from the right ventricle, across the annulus, and into the main pulmonary artery. While this creates pulmonary regurgitation, it allows for growth and improved right heart function. Many patients eventually require pulmonary valve replacement as adults due to progressive right heart enlargement from chronic regurgitation.
