Full Definition

Periodic lateralized epileptiform discharges (PLEDs) are repetitive sharp waves, spikes, or sharp-and-slow wave complexes that occur at regular intervals, typically every 0.5-3 seconds, and are lateralized to one hemisphere. PLEDs are considered an ictal-interictal continuum pattern and often indicate acute destructive brain lesions such as stroke, encephalitis, or tumor. They may be associated with clinical seizures or represent a form of non-convulsive status epilepticus. The pattern is important for prognosis and treatment decisions in critically ill patients.
