Secondary alveolar bone grafting (SABG) is a reconstructive procedure typically performed between ages 9-11 years to fill alveolar cleft defects with bone graft material. The timing coincides with the mixed dentition phase, specifically before eruption of the permanent canine tooth. The procedure aims to provide bone support for tooth eruption, create continuity of the dental arch, provide nasal floor support, and facilitate orthodontic tooth movement. Cancellous bone, usually harvested from the iliac crest, is placed into the alveolar cleft site. Success rates are high when performed at the optimal time, and the procedure significantly improves long-term dental and facial development.
