Vertical maxillary excess (VME) is a dentofacial deformity characterized by an overgrowth of the maxilla in the vertical dimension, leading to excessive gingival display during smiling and an elongated lower facial height. Patients with VME typically present with a 'gummy smile' where more than 3-4mm of gingiva is visible during animated smiling, along with lip incompetence and a long face syndrome. The condition can be isolated or associated with other skeletal abnormalities. Treatment typically involves superior repositioning of the maxilla through Le Fort I osteotomy with impaction, which simultaneously reduces gingival show, improves lip competence, and restores facial proportions. The degree of maxillary impaction must be carefully planned to maintain proper dental show and avoid creating other aesthetic or functional problems.
