Intraosseous anesthesia involves perforating the cortical plate of the alveolar bone and injecting local anesthetic solution into the medullary (cancellous) space immediately surrounding the target tooth. It is widely used as a supplementary technique when conventional infiltration or block anesthesia provides incomplete pulpal anesthesia, particularly in mandibular posterior teeth. Onset is rapid—typically within 30 seconds—because the cancellous bone is richly vascular. Editors must distinguish between primary intraosseous delivery and conventional infiltration; the two are not synonymous. The Stabident and X-Tip systems are common proprietary devices associated with this technique.
