Subscriber
The individual in whose name a dental insurance policy is held, who may also be referred to as the primary insured or policyholder.
Full Definition
The subscriber is the person who holds the dental insurance contract with the carrier—typically an employee who obtains coverage through an employer-sponsored benefit plan. Dependants covered under the same plan (a spouse or children) are distinct from the subscriber. On dental claim forms, the subscriber's identification number, group number, and date of birth are required fields. Editors must be careful to distinguish 'subscriber' from 'guarantor' (the financially responsible party) and from 'patient' (the individual receiving treatment), as these three roles may be fulfilled by different people and have specific fields on ADA claim forms and insurance correspondence.
