A superbill is a comprehensive encounter document combining elements of a receipt and a claim form. In the dental context it lists the date of service, provider NPI, patient information, CDT procedure codes, ICD diagnostic codes where applicable, submitted fees, and any amounts collected. Patients typically request a superbill when the dental practice does not file claims on their behalf—common in out-of-network or fee-for-service settings—so they can submit for reimbursement themselves.
