Paradoxical outward movement of myocardial wall during systole, indicating transmural myocardial infarction.

Full Definition

Dyskinetic wall motion describes the abnormal outward movement or bulging of the myocardial wall during systolic contraction, when it should normally move inward. This paradoxical motion typically indicates transmural myocardial infarction with formation of a ventricular aneurysm. Unlike akinetic segments which simply don't move, dyskinetic areas actively move in the wrong direction during systole. This finding carries significant prognostic implications and may require surgical intervention such as aneurysmectomy in appropriate candidates.
