Full Definition

Intraoperative parathyroid hormone (iPTH) monitoring is a biochemical technique that measures circulating PTH levels in real-time during parathyroid surgery. Blood samples are typically drawn before incision, before gland removal, and at 5-10 minute intervals after excision. A greater than 50% decrease in PTH levels from the highest pre-excision value, with levels falling into the normal range, indicates successful removal of all hyperfunctioning parathyroid tissue. This monitoring is particularly valuable in minimally invasive procedures and helps surgeons determine when to terminate the operation, reducing the risk of persistent hyperparathyroidism.
