Intraoperative nerve monitoring (IONM) involves continuous electrophysiological assessment of recurrent and superior laryngeal nerve function during thyroid and parathyroid surgery. The system uses specialized endotracheal tubes with surface electrodes that detect muscle responses when nerves are stimulated. This technology helps surgeons identify nerve location, assess nerve integrity in real-time, and modify surgical technique to prevent nerve injury. IONM has become standard practice in many centers, particularly for revision surgery, substernal goiters, or cases with increased risk of nerve injury. The technology requires proper setup, interpretation of signals, and understanding of potential false-positive and false-negative results.
