Normocalcemic hyperparathyroidism is a form of primary hyperparathyroidism characterized by persistently elevated or inappropriately normal parathyroid hormone levels in the presence of consistently normal serum calcium concentrations. This condition is increasingly recognized as an early or mild form of primary hyperparathyroidism. Patients may experience symptoms typical of hyperparathyroidism including bone disease, kidney stones, or neurocognitive symptoms despite normal calcium levels. The diagnosis requires careful exclusion of secondary causes of elevated PTH and consideration of vitamin D status.
