The technique capitalizes on the selective uptake and retention of sestamibi by hyperfunctioning parathyroid adenomas to provide real-time localization during surgery. The gamma probe detects areas of increased radioactivity, guiding the surgeon to the abnormal gland and confirming complete removal by demonstrating significant count reduction after excision. This method is particularly useful for ectopic glands, reoperative cases, or when multiple imaging modalities have failed to localize the adenoma.
