Senescent myocardium refers to cardiac muscle tissue that has undergone age-related cellular changes including decreased contractility, increased fibrosis, reduced regenerative capacity, and altered metabolic function. These changes occur at the cellular level through mechanisms such as telomere shortening, mitochondrial dysfunction, and accumulation of oxidative damage. In geriatric cardiology, understanding senescent myocardium is essential for interpreting diagnostic tests, predicting treatment responses, and setting realistic therapeutic goals. The concept helps explain why elderly patients may have preserved ejection fraction but still experience heart failure symptoms.
