Cognitive fluctuations refer to spontaneous alterations in cognitive function, attention, and alertness that occur over hours, days, or longer periods. These fluctuations are a hallmark feature of dementia with Lewy bodies but can also occur in other neurocognitive disorders and delirium. Patients may appear alert and cognitively intact at one time, then confused and disoriented shortly afterward. These variations can significantly impact functional assessment and care planning.
