Paradoxical disinhibition describes the counterintuitive phenomenon where elderly patients experience increased agitation, confusion, or behavioral disturbances after receiving medications intended to have calming or sedating effects. This reaction is particularly common with benzodiazepines, antihistamines, and some sleep medications in geriatric populations. The mechanism involves complex interactions between age-related changes in drug metabolism, altered neurotransmitter sensitivity, and underlying cognitive impairment. Rather than the expected sedation, patients may exhibit increased restlessness, confusion, combativeness, or inappropriate behavior. This reaction is more frequent in patients with dementia, delirium, or other cognitive impairments. Recognition of this phenomenon is crucial for medication safety and often requires discontinuation of the offending agent and consideration of alternative, non-pharmacological interventions.
