Ureterolysis is the surgical technique of carefully dissecting and mobilizing the ureter from surrounding tissues to clearly identify its course and prevent inadvertent injury during pelvic surgery. This procedure is particularly important in cases of severe endometriosis, large fibroids, ovarian masses, or radical hysterectomy where normal anatomy may be distorted. The technique involves identifying the ureter at the pelvic brim, following its course through the parametrium, and creating a plane of dissection that allows safe retraction. Proper ureterolysis reduces the risk of ureteral injury, which is a serious complication of gynecologic surgery.
