A rectovaginal examination is a specialized component of the pelvic examination in which the examiner places one finger in the vagina and another finger in the rectum simultaneously to palpate and assess pelvic structures. This bimanual technique provides enhanced evaluation of the posterior pelvis, including the uterosacral ligaments, pouch of Douglas, rectovaginal septum, and posterior uterine wall. The examination is particularly valuable for detecting endometriosis, evaluating pelvic masses, assessing uterine position and mobility, and investigating symptoms such as pelvic pain or suspected pathology in the posterior compartment. While uncomfortable for patients, this examination provides diagnostic information that cannot be obtained through vaginal examination alone.
