The quadriplegia test, also known as the intrinsic function test, evaluates the integrity of intrinsic hand muscles by assessing the ability to extend the interphalangeal joints while the metacarpophalangeal joints are held in flexion. The test is named because it simulates the hand position commonly seen in quadriplegic patients where extrinsic extensors function but intrinsics are paralyzed. When intrinsic muscles are intact, patients can actively extend the IP joints from this position. Inability to perform this movement suggests intrinsic muscle weakness or paralysis, which has significant implications for hand function and surgical planning.
