Rolando fracture

Comminuted intra-articular fracture at the base of the first metacarpal, more complex than a Bennett fracture.

Full Definition

A Rolando fracture is a comminuted intra-articular fracture involving the base of the first metacarpal bone at the carpometacarpal joint. This injury represents a more severe and complex variant of the Bennett fracture, characterized by multiple fracture fragments creating a 'T' or 'Y' shaped pattern on radiographic imaging. The fracture typically involves three or more fragments, making reduction and fixation technically challenging. Due to the comminuted nature and joint involvement, Rolando fractures carry a poorer prognosis than Bennett fractures and are associated with higher rates of post-traumatic arthritis and functional impairment. Treatment may require open reduction with plate fixation, external fixation, or even primary arthrodesis in severely comminuted cases.
