Sagittal band rupture involves tearing of the fibrous structures that maintain extensor tendon position over the metacarpophalangeal joint. This injury commonly occurs at the long finger and results in tendon subluxation or dislocation into the intermetacarpal valleys. The condition may be acute from trauma or chronic from repetitive stress. Surgical repair involves reconstruction of the sagittal band complex to restore normal extensor tendon mechanics and prevent recurrent instability.
