Full Definition

Osteoradionecrosis is a serious late complication of radiation therapy characterized by necrosis of irradiated bone tissue, most commonly affecting the mandible in head and neck cancer patients. The condition results from radiation-induced damage to blood vessels and osteoblasts, leading to compromised bone healing and increased susceptibility to infection. Risk factors include high radiation doses, dental extractions post-radiation, poor oral hygiene, and continued tobacco use. The condition can present with pain, exposed bone, pathological fractures, and secondary infection. Prevention involves pre-radiation dental evaluation and careful post-treatment oral care, while treatment may require surgical debridement, hyperbaric oxygen therapy, or reconstruction.
