Full Definition
Twin-to-twin transfusion syndrome (TTTS) is a severe complication occurring in 10-15% of monochorionic diamniotic twin pregnancies. It results from unequal sharing of blood flow through placental vascular anastomoses, creating a donor twin (who becomes anemic and growth-restricted) and a recipient twin (who becomes polycythemic and may develop heart failure). The condition is staged according to Quintero criteria, ranging from Stage I (oligohydramnios in donor sac, polyhydramnios in recipient sac) to Stage V (fetal death of one or both twins). Treatment options include amnioreduction, laser photocoagulation of anastomoses, or selective fetal reduction.
