In psychiatric documentation, Axis I disorders represented the main clinical disorders requiring treatment, as opposed to personality disorders (Axis II) or medical conditions (Axis III). Though DSM-5 eliminated the multiaxial system, the term persists in clinical notes and treatment records. Editors must distinguish this from Axis II when reviewing historical or legacy documentation.
