pneumoperitoneum syndrome

Full Definition

Pneumoperitoneum syndrome refers to a constellation of symptoms that can occur during or after laparoscopic surgery due to the absorption of CO₂ used for insufflation. Symptoms may include shoulder pain (referred pain from diaphragmatic irritation), nausea, vomiting, abdominal discomfort, and in severe cases, hypercarbia leading to respiratory acidosis. The syndrome is caused by the systemic absorption of CO₂ through the peritoneal surface, which is enhanced by factors such as prolonged surgery time, high insufflation pressures, and increased peritoneal surface area. Most symptoms are mild and self-limiting, but monitoring of end-tidal CO₂ and arterial blood gases may be necessary in high-risk patients.
