Intraoperative cytology is a rapid diagnostic technique used during neurosurgical procedures to provide immediate microscopic evaluation of tissue samples. This method involves preparing cellular smears or touch preparations from biopsy specimens, which are then stained and examined microscopically within minutes of tissue removal. The technique allows neuropathologists to provide preliminary diagnoses that can guide surgical extent, determine adequacy of tissue sampling, and identify critical features such as tumor grade or the presence of normal brain tissue. While faster than frozen section preparation, intraoperative cytology requires specialized expertise in cytomorphology and is particularly valuable in distinguishing reactive changes from neoplastic processes.
