Thin, thread-like structures composed of hyperphosphorylated tau protein that accumulate in the neuropil (the dense network of neural processes between cell bodies) in various neurodegenerative diseases, particularly Alzheimer's disease. These threads represent degenerating axons and dendrites that have accumulated abnormal tau protein in a hyperphosphorylated state, leading to disruption of normal cellular transport mechanisms. Neuropil threads are best visualized using tau immunohistochemistry and are considered one of the major pathological lesions in Alzheimer's disease, along with neurofibrillary tangles and neuritic plaques. Their density correlates with cognitive impairment severity and provides important staging information for neurodegenerative disease assessment.
