Full Definition

Cortical laminar necrosis represents selective neuronal death that typically affects layers 3, 5, and 6 of the cerebral cortex, most commonly following ischemic injury. On MRI, this appears as characteristic T1 hyperintensity along the cortical ribbon, typically becoming apparent days to weeks after the initial insult. The T1 shortening is thought to be due to lipid-laden macrophages and protein denaturation within necrotic tissue. This finding is most commonly seen following stroke but can also occur with other causes of cortical injury such as status epilepticus, hypoglycemia, or carbon monoxide poisoning.
