Delayed cerebral ischemia; neurological deterioration occurring days after subarachnoid hemorrhage, often due to vasospasm.

Full Definition

Delayed cerebral ischemia (DCI) is a clinical syndrome of neurological deterioration that occurs typically 4-14 days after aneurysmal subarachnoid hemorrhage. It manifests as new focal neurological deficits, decrease in consciousness, or both, and may be associated with cerebral vasospasm, though other mechanisms including microthrombosis and spreading depolarization may contribute. DCI is a major cause of morbidity and mortality in SAH patients. Diagnosis requires exclusion of other causes such as rebleeding, hydrocephalus, or metabolic disturbances. Prevention and treatment strategies include nimodipine, euvolemia maintenance, and induced hypertension.
