Gonadotroph adenomas are benign pituitary tumors originating from gonadotropin-producing cells (gonadotrophs) that normally secrete luteinizing hormone (LH) and follicle-stimulating hormone (FSH). Despite their cellular origin, most gonadotroph adenomas are clinically non-functioning, meaning they do not cause hormonal excess syndromes, though they may secrete intact gonadotropins or their subunits at levels insufficient to cause clinical symptoms. These adenomas typically present as macroadenomas (≥10mm) with mass effect symptoms including headaches, visual field defects, and hypopituitarism due to compression of normal pituitary tissue. Immunohistochemistry reveals positive staining for LH, FSH, or their common α-subunit. Treatment usually involves surgical resection, with radiotherapy reserved for cases with residual tumor growth or inability to achieve complete surgical removal.
