B-cell depletion therapy

B-cell depletion therapy involves the use of monoclonal antibodies, typically targeting CD20 or other B-cell surface markers, to eliminate B-lymphocytes from the peripheral circulation and tissues. In neuroimmunology, this approach has shown efficacy in multiple sclerosis, neuromyelitis optica spectrum disorder, and other autoimmune neurological conditions. The therapy works by reducing antibody production, antigen presentation, and pro-inflammatory cytokine release by B-cells. Common agents include rituximab, ocrelizumab, and ofatumumab. Treatment requires monitoring for hypogammaglobulinemia, increased infection risk, and potential for progressive multifocal leukoencephalopathy.
