CSF-specific oligoclonal bands represent immunoglobulin G antibodies that are synthesized within the central nervous system and are detectable in cerebrospinal fluid but not in the corresponding serum sample. This pattern, identified through isoelectric focusing and immunoblotting, is highly suggestive of ongoing inflammatory processes within the CNS. CSF-specific bands are distinguished from systemic bands (present in both CSF and serum) and are considered more diagnostically significant for conditions like multiple sclerosis. The presence of CSF-specific oligoclonal bands supports the diagnosis of inflammatory CNS diseases and indicates breach of the blood-brain barrier with local immune activation.
