Intrathecal synthesis refers to the local production of immunoglobulins within the central nervous system, distinct from antibodies that cross the blood-brain barrier from systemic circulation. This process is detected by comparing CSF and serum antibody levels and calculating indices such as the CSF/serum albumin ratio and IgG index. Intrathecal synthesis is a hallmark of neuroinflammatory diseases like multiple sclerosis, where it manifests as oligoclonal bands in CSF that are absent from serum. The presence of intrathecal synthesis indicates active immune responses within the CNS compartment.
