Kappa free light chains are components of immunoglobulins that can be measured independently in cerebrospinal fluid and serve as sensitive markers of intrathecal B-cell activation and antibody synthesis. The kappa/lambda ratio in CSF is elevated in inflammatory CNS conditions, particularly multiple sclerosis, where it may be more sensitive than traditional oligoclonal band testing. Free light chain analysis provides quantitative assessment of B-cell activity within the central nervous system and can be useful for monitoring disease activity and treatment response. This biomarker is increasingly recognized as a valuable addition to the diagnostic workup of suspected inflammatory neurological conditions.
