Polarity Reversal

The programming technique of switching electrode polarity (anode/cathode) to modify stimulation field direction and coverage patterns.

Polarity reversal is a programming technique in neuromodulation where the electrical polarity of stimulation electrodes is switched, changing anodes to cathodes and vice versa. This technique can dramatically alter the direction and shape of electrical fields, potentially improving therapeutic coverage or reducing side effects without changing electrode positions. Polarity reversal is particularly useful when stimulation coverage needs to be shifted to different anatomical regions or when unwanted muscle contractions or paresthesia need to be eliminated. The technique takes advantage of the fact that neuronal activation typically occurs near cathodes (negative electrodes), so reversing polarity can redirect stimulation effects. Modern programmers allow easy polarity switching as part of routine optimization procedures.
